If you would like to make an appointment, please contact our office by phone or email. Our office facilitates communications and emergency calls and appointments are available and welcome, especially for new patients seeking help.
If you are unable to keep an appointment, we ask that you kindly provide us with at least 48 hours notice. We ask for this advance notice so that we can offer this appointment to another patient. A fee may be charged if a patient does not show up for an appointment without sufficient notice.

Insurance and Billing
We will do everything we can to maximize your insurance reimbursement for covered procedures. If you are a new patient, you can expedite your reimbursement by bringing your insurance ID card and completed Registration Forms to your first appointment.

If you have any questions about fees and coverage prior to your treatment, please call us. Since insurance plans are individually written, however, we will only be able to ESTIMATE your share of the cost. You will be responsible for payment of any costs not covered by your insurance company at the time of your visit.

Accepted insurance coverage
We file on most dental insurance, including indemnity and PPO plans. We are in-network providers for the following companies: Delta Dental Premier, Assurant, Aetna and Cigna.

Payment Terms
We are committed to providing you and your family with the best possible dental care. By adhering to our payment policy, you can help us achieve that goal. For your convenience, a complete, downloadable version of the Payment Policies is now available through this website.

Accepted Forms of Payment
Payment is due at the time of service. We accept cash, check, debit cards and credit cards (American Express, Mastercard, Visa, Carecredit, and Discover) as forms of payment. If you have approved dental insurance, we will be pleased to help you receive your maximum allowable benefits.

Missed Appointment Charge
Accounts may be charged a broken appointment fee for any missed appointments without at least 48 hours notice. Once your appointment has been scheduled, please remember that this time has been reserved specifically for you. Giving advanced notice of a cancellation is not only courteous, it also helps us to better serve the needs of you and other patients.

Service Fees
If you still have an outstanding balance after all insurance adjustments have been made, a finance charge of 1.5% per month (18% APR) will be applied to your account when 60 days or more past due. There is a $25 fee for returned checks. Any legal fees incurred in the process of collecting payment are payable by the patient.
